rTMS for Tinnitus

چکیده
I applaud the authors of the article “Can temporal repetitive transcranial magnetic stimulation be enhanced by targeting affective components of tinnitus with frontal rTMS? a randomized controlled pilot trial” because many of them are pioneers in the development of protocols that use rTMS to treat chronic tinnitus. Investigators (including myself) around the world conducting similar studies of rTMS for tinnitus often follow the procedural examples of these researcher–clinicians in Germany. In this recent publication, Kreuzer et al. (2011) compared the effects of 2000 daily pulses of rTMS delivered to patients’ left temporal region with a protocol that began with 1000 pulses of right frontal rTMS followed by 1000 pulses of left temporal rTMS. In both protocols, patients exhibited improvement in measures of tinnitus severity. However, there was no significant difference in treatment outcomes between the left temporal and the combined (right frontal + left temporal) rTMS protocols. I would like to add a few comments about this study:

rTMS Induced Tinnitus Relief Is Related to an Increase in Auditory Cortical Alpha Activity
Chronic tinnitus, the continuous perception of a phantom sound, is a highly prevalent audiological symptom. A promising approach for the treatment of tinnitus is repetitive transcranial magnetic stimulation (rTMS) as this directly affects tinnitus-related brain activity. Several studies indeed show tinnitus relief after rTMS, however effects are moderate and vary strongly across patients. This ...
متن کاملTheta, alpha and beta burst transcranial magnetic stimulation: brain modulation in tinnitus
INTRODUCTION Some forms of tinnitus are considered to be auditory phantom phenomena related to reorganization and hyperactivity of the auditory central nervous system. Repetitive transcranial magnetic stimulation (rTMS) is a non-invasive tool capable of modulating human brain activity, using single pulse or burst stimuli. Burst rTMS has only been performed in the theta range, and has not been u...
متن کاملTranscranial magnetic stimulation and tinnitus: implications for theory and practice.
T innitus remains an enigma, with no consensus on the pathophysiology or treatment of this condition. Transcranial magnetic stimulation has recently emerged as a possible treatment modality that has shown some promise. The paper by Plewnia et al, (see p 152) reported on the efficacy of delivering 10 sessions of low-frequency repetitive transcranial magnetic stimulation (rTMS) over the temporopa...
متن کاملShort Term Effects of Repetitive Transcranial Magnetic Stimulation in Patients with Catastrophic Intractable Tinnitus: Preliminary Report
OBJECTIVES The short-term effects of low-frequency repetitive transcranial magnetic stimulation (rTMS) in the patients with catastrophic and intractable tinnitus were investigated. METHODS Fifteen participants were recruited among patients with catastrophic intractable tinnitus to receive 1 Hz rTMS treatment. Tinnitus severity was assessed before rTMS and directly after sham or real rTMS usin...
متن کاملRepetitive Transcranial Magnetic Stimulation Treatment for Chronic Tinnitus: A Randomized Clinical Trial.
IMPORTANCE Chronic tinnitus negatively affects the quality of life for millions of people. This clinical trial assesses a potential treatment for tinnitus. OBJECTIVES To determine if repetitive transcranial magnetic stimulation (rTMS) can reduce the perception or severity of tinnitus and to test the hypothesis that rTMS will result in a statistically significantly greater percentage of respon...
متن کاملRepetitive transcranial magnetic stimulation induces oscillatory power changes in chronic tinnitus
Chronic tinnitus is associated with neuroplastic changes in auditory and non-auditory cortical areas. About 10 years ago, repetitive transcranial magnetic stimulation (rTMS) of auditory and prefrontal cortex was introduced as potential treatment for tinnitus. The resulting changes in tinnitus loudness are interpreted in the context of rTMS induced activity changes (neuroplasticity). Here, we in...
